About Us

CAARN was funded through an NIH grant, with additional support from the UW-Madison Institute for Clinical and Translational Research and the School of Medicine and Public Health.

NIH funding helped CAARN expand to better serve African American communities and to add health care system partners with a focus on improving health for those with multiple chronic conditions.
